The questionnaire will take the form of a HTML form.
Each field will be linked to some javascript functions (using onChange handlers - onClick for radio buttons and checkboxes).
The scripts will load images as necessary to display. Initially there will be one image (in a separate frame to the questionnaire) which will change.
Sounds must be included in the questionnaire document using the <embed> tag.
Flash movies can be loaded/triggered as necessary
